Chapters devoted to projects that received LFP matching grants:
- 2008 - Volume XI (chapter 5) highlights a successful former LFP project in Fort Collins, CO. Read about Community Mental Health and Substance Abuse Partnership and their strong collaboration among more than 30 nonprofit and public agencies. Plus enjoy the story of how the program began in our LFP Storybook.
- 2007 - Volume X (chapter 7) focuses on SPARC (Sickness Prevention Achieved through Regional Collaboration), a project serving residents at the junction of Connecticut, Massachusetts and New York. Among many new strategies, SPARC pioneered the Vote & Vaccinate program to offer flu shots to seniors at polling places on election days.
