When an individual with chronic mental illness experiences an after-hours crisis, the choice has been jail or the emergency room. Now the Milwaukee Crisis Resource Center offers respite housing and stabilization staffed by peer support specialists and an on-site nurse. The Center provides mental and physical health assessment, case management and connections to existing housing, mental health, and legal services. A community mental health task force including the police, courts, county mental health, hospital systems, mental health service providers and consumers, family members and advocates developed this remarkable public/private partnership.

Pediatric Behavorial Medicine and Health Services
Neenah, WI
In this promising service delivery model, pediatric psychiatrists partner with primary care providers to increase mental health services for underserved children in a rural, seven-county region of Wisconsin. For childhood disorders such as depression, ADHD and anxiety, a psychiatrist will evaluate a child and then coordinate ongoing care with the local pediatrician. In a significant systems change, two local health systems will open their mental health services to all children regardless of their insurance affiliation. The project staff will reach out to the most vulnerable populations including Hmong, Latino and Native American families.

Latino Geriatric Center
Milwaukee, WI
Access, language and socio-cultural barriers can prevent the diagnosis of dementia in Spanish-speaking patients and hinder efforts to find culturally sensitive care. This new Latino Dementia Center is located in a vibrant Hispanic community service center with a charter school, restaurant, fitness and arts centers as well as an established senior center and adult day program. Participants with Alzheimer''s will receive medication management, adult day services and caregiver support through partnership with two medical schools and the local Alzheimer's association. Outreach campaigns will inform the Latino community of the benefits of early diagnosis and encourage physicians to refer Hispanic patients to the program.

Madre Angela Dental Clinic

Provides oral health care to homeless and impoverished people in Milwaukee through a collaboration of volunteers, students and staff. The project will expand the clinic to provide access to urgent dental care, provide dentures and restorative appliances and provide pediatric dental care and sealants.
